This is a version with minimal permissions required, no direct access to Internet and no ads. A demo version is available on Google Play - search for "jnmwsi" and select Diary Notes Multimedia.
Permissions information:
Modify USB and SD card contents - required to export/backup the notes on the SD card and for the storage of the media files
Vibrator - used for reminder notifications
